New Patient Information
We try to make the new patient process as easy and simple as possible, so that we may begin your care in a timely manner. Please print out the new patient intake forms below, in the left-hand column. A normal new patient visit can take 45 minutes to an hour. By filling out these forms in their entirety; you can decrease that time by 15-20 minutes.
Also, to show our appreciation to our new patients, we have provided new patient coupons. You may print out these coupons from the right-hand column below for discounts in and around Cape Coral, including Houlihan’s Restaurant and the Run Shoppe. Simply bring them in with you at your first visit for validation, and enjoy! |
